Health History Consultation

Scheduling a FREE Health History Consultation is the best way to get started. This is a great way for me to get to know you, and you to get to know what working with me would be like. This also gives us the space to be able to answer any questions you might have at this point.

The Health History Consultation takes up to 45 minutes, and is done via the Facebook Planta Vita LLC page, so be sure to follow beforehand. Once your appointment is booked, I will send you a Health History Form to your email to be filled out at least an hour prior to your appointment. The form will ask you some questions about your current health, past health and family history. It’s important for me to be able to know where you’re coming from so that I can meet you where you are and help you move forward.
